Accessing Your Health Care Records
To request your records, please:
Submit a written request to Greater Houston Counseling Services
Include your full name, date of birth, and a brief description of the records you are requesting
Let us know how you would like to receive your records (secure electronic delivery or mail)
Our team is happy to assist you with this process, and all requests are handled in accordance with applicable state and federal privacy laws.
